Where your health data goes with ChatGPT Health and Perplexity
ChatGPT Health (OpenAI) and Perplexity Health are real products that connect to Apple Health on your iPhone. They read your HealthKit data and process it on their servers to generate insights and answer questions about your health trends.
That processing happens under each company's consumer privacy policy, not HIPAA. That's a meaningful difference in how your data can be used, retained, and protected. Some people are comfortable with that trade-off. Others aren't. Neither position is unreasonable.

What about the built-in AI chat?
To be direct: the Pro tier's in-app AI chat does send data off your device. When you ask a question in the chat, the relevant portion of your health data is sent to Google Gemini to generate a response.
Two things worth knowing. First, data is only sent during an active chat session — not in the background, not automatically. Second, Google is contractually barred from using API data to train its models, which is a different data relationship than using Gemini directly through the consumer app.
If your goal is zero data leaving the iPhone, Free and Premium are the right choice. Export your CSV locally, then decide which AI tool (if any) you want to use with it. The Pro tier is for people who want the convenience of in-app AI chat and are comfortable with the Gemini data relationship.
